These photos of observed violations are intented to educate employees with regard to the common "what not to do" scenarios. We can all learn from past mistakes to improve safety for everyone.

This waste bottle was carelessly left open. All waste bottles should remained closed when waste is not being added. .BDE

This waste container is not labeled at all as to its contents.The container also lacks a waste start date and the words "Hazardous Waste." .BDE

Arrows indicate solvent bottles stored under a sink in a laboratory. Under no circumstances should chemicals be stored underneath sink cabinets. .BDE

This photos shows an improperly labeled waste conainer (red) and several botles of flammable reagents stored on a cart for an extended period of time (blue). Flammable reagents should be stored in flammable cabinets.BDE

Shown is a half-full bottle of hexanes found stored on the floor. At no time should bottles of reagents/solvents be stored on the floor.BDE

This shelf is leaning dangerously, with large, heavy, and dangerous items stored on the top shelf. Shelf units in disrepair should never be used and heavy items should not be stored at the top.BDE
This eyewash and safety shower station is blocked by a large object. To prevent wasting precious time in an emergency, keep all safety shower and eyewash station clear of obstacles.BDE

This photo shows a refriderator and freezer storing both food for human consumption and chemicals/samples. These items should never coexist in the same areas.BDE

These gas cylinders(2 acetylene, 1 oxygen) are left unsecured and unchained to a wall or bench. To prevent accidents, all cylinders should be chained to a stable, sturdy object or wall.BDE
